By filling out the info below and hitting the “Submit“ button, a Life Church staff member will send you a “teamLIFE” APPLICATION by email or by actual postal mail. We don’t mind sending you the application by email. However, for your privacy, we suggest you fill out the application and bring it back
to the welcome center on a Sunday or Wednesday, or to the office Tues-Fri 8am-3pm.
